What is Case Management?

Case management helps people with complex chronic health conditions navigate the health care system, coordinate health care, and get support to meet their health care goals.

RIPCPC’s Case Management team of nurses, pharmacists, and community health workers connect you to the health care services and resources that you need to stay healthy. You and your RIPCPC nurse case manager will create a personalized care plan for you to manage your health needs. You will receive help to understand your medical conditions, get referrals to community resources, schedule your appointments, and manage your medications.
