Each patient has their own doctor who then leads a care team that collectively assist in providing comprehensive and continuous medical care to patients with a goal of obtaining improved health outcomes.
The 'center' of the medical home is the patient. 'Patient-centered' means considering patients’ cultural traditions, personal preferences, values, family situations, social circumstances and lifestyles. Our PCMH practices have same day appointments reserved so that you can been seen when health concerns arise. Unless you are experiencing a medical emergency, you should call your primary care provider before going to the hospital. They can assist with many urgent conditions and services.
On an annual basis, PCMH practices collect patient satisfaction surveys to gather important feedback and data and to improve service and care as needed.
PCMH practices receive and review clinical quality data on an ongoing basis. They utilize clinical data to establish performance goals and identify patients who are due for preventive services, outreaching to patients as needed.
