Annual Physicals

Having a yearly physical exam with your primary care physician is one of the most important things you can do for your health. These annual check-ups allow your doctor to assess your overall health, address concerns or symptoms you may be experiencing, and prevent conditions or illnesses before they arise. When you establish care with one of our physicians, they will get to know you and help you manage your health and wellness goals, concerns and risk factors.

Same Day Appointments

When you experience a health concern that can’t wait, our physicians are ready to accommodate you. Our doctors reserve same-day appointments for nonemergency care needs so that you can contact your physician and be evaluated that same day. In fact, many conditions that patients go to the emergency department for, such as minor injuries, can be treated by your primary care doctor. Our same-day appointments can help you avoid a trip to the hospital, saving you time and money.  

Telehealth 

If you can’t meet with your doctor in-person, you have the option to schedule a virtual appointment so that you can meet with them from your smartphone or computer. Telehealth is available for most types of appointments, ranging from chronic care and follow-ups to sick visits and your doctor will provide you with the information you need to join the video call.

Immunizations and Preventive Screenings

Keeping up with recommended immunizations and screenings will help you prevent certain types of illness. Your doctor will work with you to provide immunizations to protect from certain diseases and age appropriate preventive screenings that will assess your risk and detect chronic conditions before they develop.

Care Management

When you need it, our clinical care teams will help you manage your health needs and conditions at no cost to you. Our team of nurses, pharmacists, and social workers work with your doctor to develop treatment plans, provide support services, and connect you with community resources.

Care Coordination

We have a vast network of local specialists that our doctors have partnered with to better coordinate your care. When you need to see a specialist, our team of referral coordinators will manage your referral and assist you in scheduling an appointment.

After Hours Care

Scheduled appointments for both urgent and routine care outside of normal business hours are available at convenient locations at some of our affiliated clinics.
